Episode Description:
DOUBLE TROUBLE — Four years ago, Sam (Jared Padalecki) and Dean (Jensen Ackles) helped a woman named Nora (guest star Nicole Oliver) exorcise a demon that was systematically killing women in a small town. They managed to save the postal worker, Jeffrey (guest star Russell Sams), while vanquishing the demon but now it seems the black-eyed monster has reared his ugly head and returned for a second helping. The brothers visit Jeffrey, who has been locked in a psych ward since the incident, to see if he can remember any of the demon’s plans while he was possessed.
Thomas J. Wright directed the episode written by Ben Edlund.

TVLINE | Mark Pellegrino is coming back as Lucifer, so what’s going on upstairs with Sam?
A lot. Though [Lucifer] hasn’t been seen on camera a lot, he’s been the voice in Sam’s head since the first episode [of the season], when he was messing around with Sam. Sam’s just been able to hide it, but an episode comes up where Sam has no choice but to confront this voice in his head. That makes it much more difficult for Sam to then get rid of Lucifer once he has embraced his presence.
TVLINE | Is it going to leave Sam in a worse place?
Very much. The fact that Sam’s been able to ignore him and push his scar and send him away has kept his hallucinations and his craziness at bay. But once he engages Lucifer — or his vision of Lucifer — then Lucifer almost… It’s like inviting a demon into your house. Once you invite him, you can’t get rid of him. So we see Sam suffer the repercussions of having engaged in his vision of Lucifer.

The physical scar has to go away, so what will keep him knowing for sure that he’s in reality?
J.P.: It’s going to go away soon. And quite frankly where we are in the season [shooting] now is he loses everything. He loses all– what’s that movie with Leonardo DiCaprio where they spin the top?
Oh, Inception!
J.P.: Inception! Yeah! He loses his way to figure out reality and falsehood…It ends up driving him crazy. Crazy crazy where he can’t function anymore.
So Mark Pellegrino has to be coming back, but who else may make a return in Sam’s hallucinations?
J.P.: Mark’s [shooting] there right now…
J.P.: Mark Pellegrino comes back as a vision in Sam’s head. Sam finds himself in a situation where he has to decide between engaging this vision and risking the consequences or ignoring and risking other consequences involving his brother. So we’ll see what happens.
